2010-2011 Registration Forms
Directions for Registration
Parents, please take time to carefully fill out the necessary forms by typing in your information

Directions: 
1.    Before starting, make sure that you have Adobe Acrobat Reader software installed (free to download here) and that you are on a                computer with a working printer attached.

2.    As you click on a title of a document below, a .pdf file will open.  It is important (and unfortunate) to note that you will not be              able to save the document with the information you enter into it.  Your computer should remind you of this (unless you have                Adobe Acrobat Standard or Professional installed on your computer, which most people do not).  You will need to print out the            completed form to send to school. 

3.    As you open a document, click on the "Highlight Fields" at the top right corner of the page so that you can see the fields that you          will need to fill in.  If done correctly, the fields should be highlighted blue.


4.    Use your mouse to navigate to each field in the form (tabs or returns will not necessarily take you to the next field). 


5.    When you are finished filling in the form, please print the document and sign where/if necessary.  Only one parent/guardian must          sign the forms.


6.    You should be able to click the “Back” button on your browser to come back to this webpage where you will be able to choose the        next form, but depending on the version of Adobe Reader you may have to close out.  Repeat the process for all of the forms that        you need for registration.


The completed forms need to be returned to the Assumption School’s office by May 15, 2010. 
Feel free to make a copy of these forms for your files before turning them into the office.  You may mail them in or drop them off in the school office.

Parents with a child in the Pre-School Program (Three-year old)
Parents with a child in the Pre-Kindergarten Program (Four-year old)
Parents with exactly one child in Kindergarten through Eighth grades
Parents with exactly two children in Kindergarten through Eighth grades
Parents with three or more in Kindergarten through Eighth grades

Parents who chose Electronic Funds Transfer (EFT) Payments on their Tuition Contract:
